Though I do not have any information on specific programs related to your inquiry, I did reach out on your behalf to Professor Wes Schultz, who teaches at California State University and who has extensive knowledge and expertise in social marketing and behaviour change. He referred me to the attached journal article, which is a study that tested the habit discontinuity hypothesis. This hypothesis states that "behaviour change interventions are more effective when delivered in the context of life course changes". As you can see from the article, the results indicated that the 'window of opportunity' is three months after relocation.
