There are a large number in the UK, from national schemes through to very local ones, and of course ones that are generated on a school-by-school level by passionate teachers. You will be literally swamped by how much there is out there.

Charity Global Action Plan www.globalactionplan.org.uk run a number of programmes.
In the states the Centre for Ecoliteracy is a good place to start.
CERES in Melbourne runs a number of projects as well, collingwood college has a very successful school garden.

The UK has stacks & stacks of examples as it is a requirement for schools to undertake an STP and there is government funding for each local authority to employ a school travel advisor.

There is a long standing development in California called 'Village Homes'. Their website isn't amazing, but if you can get hold of a copy of an ABC (Australia) documentary series called 'Global Gardener' about Permaculture developments world wide Village homes is featured on there.
A couple of sites with lots of resources on the areas you are talking about are:

Also check out www.permaculture.org.au and watch the 'Greening the Desert' flash movie; very inspiring stuff & covers some of the wider concepts you are interested in, perhaps for a post-urbanised world!
I can give you tips on downloading a whole raft of sustainability documents & videos; not sure what the protocols are on this listserv about promoting unliscenced content, but if i recommend www.scribd.com and www.thepiratebay.org you can figure the rest out for yourself!

Just to note as well that the UK has legislation whereby planning conditions and obligations can be put on new developments (both busineses and residential) and old developments that are seeking additions & need to undergo planning permission. Through these laws local councils can require sustainable travel infrastructure to feature in the development, and also reduce the facilities available for cars.
Worth looking into if state govts in the US have the power to require similar conditions.

There are plenty of examples out there if you do a bit of google searching. The comment that I would make is that in my experience this type of behaviour change really needs to involve engagement with the audience; empowering / facilitating motivated members of the audience to encourage their peers to take action. A web tool can support this, but I really think that travel change is something that involves working through travel options for people in a fairly customised way (there isn't really a one size fits all as people live in different places & have different needs & barriers). You also need to consider how to monitor / evaluate the effect of the website.
Have a look at the school and workplace travel planning that Transport for London do - www.anewwaytowork.org and also the DfT's Smarter Choices work. TfL has guidance on delivering travel plans for both audiences.
In terms of specific web tools somethings to consider are a carbon savings calculator & a calorie counter to act as motivators.
