We are designing a transportation survey for staff, faculty and students - modes used, barriers to using alternative modes, etc. I am looking for sample surveys and/or reports from surveys.

The College Sustainability Report Card (http://www.greenreportcard.org) surveyed over 290 post secondary institutions in the US and Canada and one category was transportation:

I have found it very useful in researching what other campuses are doing transportation, buildings/operations, food, waste management, purchasing policies/investing, and student engagement.
